I will attach a virtual copy of your insurance card to this email, but you’ll be getting a physical copy of the card within 10 business days of the policy start date. I will include the links below to access your account and all its benefits, look up providers, and the customer service line if you ever have questions on the policy.

How to search for Providers in your National PPO Network

  This will open-up a new page for you to search for providers accepting your Insurance Network and your area.

Prescriptions

  Use the discount card attached by Perks Optum, you can also visit their website and get another card via text.
  You have to give that card to the pharmacy for processing when picking-up your prescriptions.
  The Rx Benefit on your plan is a reimbursement upon submission of your full prescription receipts (not just the charge) to Claims at UHOne (address on the back of ID Card). I would suggest you do it 3-6 times per year.
  To submit your prescription claims, you can either mail, fax, or upload the Itemized Bill to your member portal. You need to ask the pharmacist each time for an itemized bill. An itemized bill is NOT the same as a customer receipt. An itemized bill includes the type of prescription, the charge/amount of medications that you paid, and the name/address/phone number for the pharmacy. In addition, you will need to include your full name, member ID number, date of birth, address, and phone number. Please allow 6-10 weeks for processing.

Scheduling appointments with the provider's office/facility:
  Please keep in mind this is a $0 copay plan. If a receptionist asks you to pay at the door they are mistaken.
  If for some reason the provider's office is not able to verify your benefits with your current coverage, you need to specify that you are insured with Golden Rule Insurance/United HealthOne.
  You are part of the UHC Choice Plus, which is the largest network in the country.
  If the provider wants to verify benefits with your Insurance carrier, they need to call the United Healthcare phone number: 800-657-8205 and press #2, then #2. This is the same number that is on the back of your Member ID Card.
  If you ever have issues with a provider that shows In-Network on UnitedHealthOne's website, and refuses to provide you with a discounted rate for services rendered, please reach out to us and we'll help you clarify this.

  MedCents is a service that will help negotiate remaining patient responsibility with providers to help make paying a little easier. You should have a final bill from the provider before seeking these services.
